site, mobile site, or any affiliated sites owned and operated by Clinical Relief Institute
LLC (collectively, the “WebTherapy Site” or “Site”). Clinical Relief Institute LLC
(“Clinical Relief Institute” or “we”) respects your privacy and provides this policy
(“Privacy Policy”) to explain what information Clinical Relief Institute may collect about
you, how Clinical Relief Institute and other companies or individuals may use it, and
which technology may be used to collect your information. This policy does not apply to
information we collect off-line, through third parties or through third party websites, or
through means other than through the AISC Websites.

When you use the WebTherapy Site, we collect information about you to provide and
enhance our products and services and improve or increase your use and enjoyment of
the WebTherapy Site. Information about you may also be shared with other individuals
and companies. This may enhance your use or enjoyment of other services or interests.
For instance, companies engaged in “targeted” or “behavioral” advertising may present
ads to you that might be of interest to you based on information about your online

WebTherapy Site, and the changes will be effective upon posting. Please check back

Information we collect

A. Information you choose to provide:
You may also choose to voluntarily provide information to us about yourself. A user
account is required to access and use the WebTherapy Site, and a profile is required for
mental health professionals to access and use the Site. By creating a WebTherapy user
account, you provide us with certain information in order to receive email notifications
from Clinical Relief Institute, use the referral function of the WebTherapy site, submit
content to the WebTherapy Site, and connect with other users. The information
provided by you may include you: (a) name; (b) personal and/or business mailing
addresses and telephone and facsimile numbers; (c) personal and business e-mail
addresses; (d) licensure, years practiced, specializations, hours of operation, and state of
licensure; and (e) credit card or bank account information (for billing and payment
purposes). You may also choose to post or provide content (such as videos, audio, text,
photographs, reviews or comments) that may be shared with others through the
WebTherapy Site or through your account with third parties. Content posted directly
on the WebTherapy Site will only show your user ID . Financial information, after
completion of the applicable transaction, will not be stored on Clinical Relief Institute
servers.

B. Information collected automatically:
When you visit or use the WebTherapy Site, we may automatically collect and store
information about your computer or mobile device and your activities so that we can
enhance our services and your experience. We may use web technologies such as cookies
and pixel tags to collect information about your visit to our WebTherapy Site. The
information we collect may include, but is not limited to: the version of our
Application/Site used; the type of your computer or mobile device (for example
PC/iPhone/Andriod/iPad); ads clicked on; coupons and/or QR codes downloaded;
internal web pages visited; searches used; IP address; browser information; and general
geographic location.

Cookies: When you visit our WebTherapy Site, we may place “cookies” on your

site-visitation statistical information through clear GIF technology which is also known
as a “web beacon”. Such aggregated statistical information is non-personal information.
This Site employs web beacons to speed your navigation of the Site, recognize each user
and his or her access privileges, and track a user’s site usage. Web beacons are small bits
of code embedded in web pages or emails that assist in delivering cookies and help the
Company determine whether a web page on this Site has been viewed and, if so, how
many times. For example, any electronic image on this Site, such as an ad banner, can
function as a web beacon. Although you cannot decline to receive web beacons when
visiting a web page, you can choose not to receive web beacons by email if you disable

Log Files: The Site server automatically recognizes the Internet URL from which you
access this Site. Clinical Relief Institute may also log your Internet protocol (“IP”)
address for system administration and troubleshooting purposes. (An IP address
indicates the location of your computer on the Internet.) Clinical Relief Institute does not
link an IP address to any personally identifiable information.

Mobile Device IDs: If you’re using our mobile Site or other software applications
(“Applications”), we may use mobile device IDs (the unique identifier assigned to a
device by the manufacturer), instead of cookies, to recognize you. We may do this to
store your preferences and track your use of the Applications. Unlike cookies, device IDs
cannot be deleted. Ad companies may use device IDs to track your use of the
Applications, track the number of ads displayed, measure ad performance and display
ads that are more relevant to you. Analytics companies may use device IDs to track
information about usage of the Applications.

C. How we use your information

Personal Information that we collect from you may be used in one or more of the

Where feasible, we require these other companies to keep our user information

We protect our user information as an asset of our business. If Clinical Research
Institute or substantially all of its assets were acquired, or in the unlikely event that
Clinical Research Institute goes out of business or enters bankruptcy, user information
would be one of the assets that may be transferred or acquired by a third party. You
acknowledge that such transfers may occur, and that any acquirer of Clinical Research
Institute may continue to use your personally identifying and non-personally identifying
information as set forth in this Privacy Policy.

Our Service Providers: We may share all of the types of information we collect with
our employees, agents, independent contractors and vendors who assist us in providing
services to you. In addition, in connection with any purchase of our products or services,
we may share information we collect from or about you with payment processors (such

collect information about our users’ computers or mobile devices and their online

and email address and what information you would like to update, correct, or delete
when you contact us.

We encourage you to promptly update your personal information if it changes. You may
ask to have the information on your account deleted or removed; however, because we
keep track of past transactions, you cannot delete information associated with past
transactions. In addition, it may be impossible for us to completely delete all of your
information because we may periodically backup information.

 E. Data Security

WHILE CLINICAL RELIEF INSTITUTE HAS TAKEN COMMERCIALLY REASONABLE
PRECAUTION TO SAFEGUARD YOUR PERSONAL INFORMATION, CLINICAL
RELIEF INSTITUTE CANNOT GUARANTEE COMPLETE SECURITY BECAUSE
COMPLETE SECURITY DOES NOT PRESENTLY EXIST ON THE INTERNET. YOU
ACKNOWLEDGE THAT CLINICAL RELIEF INSTITUTE SHALL HAVE NO LIABILITY
WHATSOEVER FOR DAMAGES OR LOSSES INCURRED BY YOU AS A RESULT OF
DISCLOSURE OF YOUR PERSONAL INFORMATION TO OTHER USERS OF THIS
SITE. YOU ALSO ACKNOLWEDGE THAT CLINICAL RELIEF INSTITUTE SHALL NOT
BE LIABLE FOR ANY SECURITY BREACH THAT RESULTS FROM CAUSES OR
EVENTS THAT ARE BEYOND CLINICAL RELIEF INSTITUTE’S CONTROL (SUCH AS
ACTS OF GOD, HACKING, TERRORISM, POWER OUTAGES, DEFECTS IN THIRD
PARTY SECURITY PRODUCTS AND SERVICES, YOUR OWN ACTS OR OMMISSIONS,
|AND OTHER SIMILAR EVENTS BEYOND THE CONTROL OF CLINICAL RELIEF
INSTITUTE). ALL CREDIT CARD INFORMATION WILL BE STORED THROUGH A
THIRD PARTY VENDOR AND IS SUBJECT TO THEIR SECURITY POLICIES.

F. Your California Privacy Rights

In the future, Clinical Relief Institute may discloses your personal information to third
parties for marketing purposes. California Civil Code Section § 1798.83 permits  California
residents to request certain information regarding the disclosure of personal
information to third parties for third party direct marketing purposes. To make such a

 G. Children’s Privacy

The WebTherapy Site is not intended for use by individuals under the age of 18. We
recognize that individuals under the age of 18 may not fully understand the
consequences of submitting personal information on our WebTherapy Site. Therefore,
Clinical Research Institute encourages parents to supervise the online activities of their
children and educate them on the responsible use of the Internet. Clinical Research
Institute does not knowingly allow individuals under age of 18 to create user accounts
and does not knowingly collect any personally identifiable information from individuals
under the age of 18. If you are under 18, do not provide any information on the
WebTherapy Site through any of the features or partake in any of the referral services.
If Clinical Relief Institute learns that it has collected or received personal information
from an individual under 18 without verification of parental consent, Clinical Relief

 H. Opt Out

If you would like to discontinue receiving emails from Clinical Relief Institute in regards
to your WebTherapy Site online account or profile or would like to close your
WebTherapy Site online account, please send an email to Help@WebTherapy.org. Please
allow 7-10 business days to stop receiving e-mails and/or push notifications.

This Privacy Policy only applies to information collected by Clinical Relief Institute. This
Privacy Policy does not apply to the practices of companies or webpages that Clinical
Relief Institute does not own or control, or employees that Clinical Relief Institute does
not manage. Any information you provide to, or that is collected by, a third party may be
subject to the privacy policies of those third parties, if any. We encourage you to read
such privacy policies of any third-party websites you visit. It is the sole responsibility of
those third parties to adhere to any applicable restrictions on the disclosure of your
personally identifying information, and Clinical Relief Institute shall not be liable for
wrongful use or disclosure of your personally identifying information by any third party,
including but not limited to disclosure by one of the mental health professionals or
individuals with whom you have connected through the WebTherapy Site.
